CODE OF CONDUCT MEETING - CANCELLED

 CODE OF CONDUCT INFORMATION

The Code of Conduct Meeting scheduled for Sunday, August 1st had been CANCELLED.

Please view the video and fill out the form before the first day of practice.

Paperwork Reminder

 

The following forms must be turned into the Business Office BEFORE the first day of practice:

Sports Form

- Including signatures relating to:

 Permit and Liability (first required signature)

Code of Conduct (must read this document first before signing)

WIAA Eligibility (must read this document first before signing)

Athletic Alternate Year/New Physical and physician's signature

Concussion Agreement Form

EVERYONE must sign this form. 

Freshmen, Juniors and anyone who did not take an IMPACT test last year will also need to have to do the baseline test.

Consent to Treat Form

Participation Fee

EVERYONE must pay a $50 athletic participation fee, this can be done through the Infinite Campus portal or paid in the Business Office -- this fee CANNOT be collected by coaches. This is separate from the team fees. 

PAPERWORK IS DUE BEFORE THE FIRST DAY OF PRACTICE!

If paperwork is not turned in by an athlete BEFORE the first day of practice, they will not be allowed to participate until their forms are turned into the office. NO EXCEPTIONS.

Paperwork CANNOT be accepted by coaches at practice.

The Appleton North Girls Swim and Dive season is scheduled to begin on Tuesday, August 10th. Those interested in participating and their parents should attend this meeting.

In addition to attending this meeting, please make sure to have all of your forms (listed under the link "REQUIRED FORMS"  in the column to the right) turned into the business office BEFORE THE FIRST DAY OF PRACTICE.
